Care Built Around Your Child and Family
You know your child best. We listen to what you have learned, what matters in your daily routines and where your family needs more support.
Before care begins, our Care Manager meets with your family in your home to complete an assessment, individualized care plan and Safety Risk Assessment. Together, we discuss your child’s care needs, how they communicate and what helps them feel comfortable. Our nurses and PSWs receive preparation specific to your child, and we stay connected as their needs change.
We also help families understand potential funding options, including Family-Managed Home Care through Ontario Health atHome, other programs and private insurance.
Familiar Faces Who Get to Know Your Child
Trust grows through familiarity. We work to build consistent care teams who get to know your child, recognize their routines and understand your family’s preferences.
Your family helps shape that team through conversations, meet-and-greets and, where appropriate, participation in interviews. We want you to feel comfortable with the person at your door and confident in the care your child receives.
Whether support gives you time to sleep, work, spend time with your other children or help your child attend school and camp, our goal is to become a trusted extension of your family.
